How the Czech Sick Leave System Works
Understanding how sick leave works in Czech Republic helps you know what to expect from this service.
- First 14 days — wage replacement from your employer: When your doctor certifies you as unfit for work, your employer pays 60% of your reduced average earnings for the first 14 calendar days. This applies to employees on standard employment contracts (HPP).
- From day 15 — sick pay from ČSSZ: If you are still unfit for work after 14 days, the Czech Social Security Administration (ČSSZ) takes over and pays nemocenská — sick pay — directly to you, for up to 380 days.
- eNeschopenka — fully electronic: Since 2020, all Czech sick leave documentation is issued electronically. When your doctor certifies your sick leave, the eNeschopenka is sent directly to your employer and ČSSZ — you do not need to deliver any paperwork. Your employer is notified automatically.
Note: To issue eNeschopenka, your doctor will need your Czech birth number (rodné číslo) and you must be registered in the ČSSZ system. Please have your birth number available before your consultation.
Who Is Entitled to Sick Leave in Czech Republic
Sick leave entitlement depends on your employment type:
- HPP — standard employment contract: Full entitlement to wage replacement (days 1–14) and nemocenská (day 15+). eNeschopenka is issued automatically.
- DPČ — dohoda o pracovní činnosti: Entitlement exists if your monthly income from this contract exceeds CZK 4,500. If so, you are enrolled in nemocenské pojištění and have the same entitlements as HPP employees.
- DPP — dohoda o provedení práce: Entitlement exists if your monthly income from this contract exceeds CZK 12,000 (2026 threshold). Below this, you are not enrolled in nemocenské pojištění and have no entitlement to wage replacement or nemocenská.
- OSVČ — self-employed: No automatic entitlement. If you voluntarily pay nemocenské pojištění (minimum CZK 243 per month), you are entitled to nemocenská from day 15. The first 14 days are not covered regardless.

What Your Consultation Includes
- Full clinical assessment: Your doctor takes a full history of your symptoms, how long you have been unwell, and the nature of your work — to make a clinical determination of whether sick leave is indicated.
- eNeschopenka — where clinically indicated: If your doctor determines that sick leave is appropriate, eNeschopenka is issued electronically. Your employer and ČSSZ are notified directly — you do not need to deliver any paperwork.
- Justification letter — where appropriate: For situations where a formal sick note is not indicated but you need documentation of your medical consultation — for example, a short absence that does not qualify for formal sick leave certification — your doctor can issue an appropriate letter at their clinical discretion.
- Referral or further investigation — where needed: If your symptoms suggest an underlying condition that needs further assessment, your doctor advises on next steps and issues any necessary referrals or investigation requests on the same day.
Important Information Before You Book
- The decision to issue sick leave certification is always a clinical decision made by the doctor after full assessment — it cannot be confirmed or guaranteed in advance
- Retroactive sick notes are not routinely issued — our doctors cannot certify sick leave for a period they did not clinically assess. If you have already been off work, discuss this with your doctor during the consultation
- To issue eNeschopenka, your doctor needs your Czech birth number (rodné číslo) — please have this ready
- DPP workers earning below CZK 12,000 per month and OSVČ without voluntary nemocenské pojištění are not entitled to sick pay — your doctor advises on what documentation is appropriate for your situation
- Our doctors do not issue sick notes without clinical justification
