Health insurance in Romania without income: documents, cost and steps in 2026

With no salary, pension or other income, you insure yourself through the Declarația unică. Which documents you need, what you pay in 2026, when cover starts and what you get if you are uninsured.

Health insurance in Romania without income: documents, cost and steps

Since August 2025, spouses and parents without income are no longer co-insured for free. Anyone with no income who is not in an exempt category insures themselves through ANAF.

To get health insurance in Romania without income, you need only a few documents: your identity document, the Declarația unică (single tax return, form 212) filed with ANAF with the option to pay CASS (the health insurance contribution), and proof that you paid the first instalment. For 2026 the contribution is 2,430 lei, which is 10% of 6 gross minimum wages of 4,050 lei. You pay 25% when you file the return and the rest by 25 May 2027. You are insured from the filing date for 12 months. You do not need to go to the health insurance house first: the return is filed in the Spațiul Privat Virtual (SPV, ANAF's online account), on e-guvernare.ro or on paper.

Who must insure themselves and who stays exempt

Before any form, check the list of exemptions. If you are on it, you are insured without paying CASS.

Since 1 August 2025, the Fiscal Code exempts the following from CASS, among others:

  • children under 18 and young people aged 18–26 who are pupils, apprentices or students;
  • young people under 26 who come from the child protection system;
  • people with disabilities, for income received under Law 448/2006;
  • cancer patients enrolled in the national health programmes, until they recover;
  • pregnant women and women who have recently given birth;
  • pensioners, for the part of their pension up to 3,000 lei a month.

Spouses and parents without income who are dependants of an insured person have been taken off the list. CNAS kept them insured until 1 September 2025. Since then, they either insure themselves or the person who supports them pays for them. Also since August 2025, CASS has been withheld at source from unemployment benefit.

Documents needed for health insurance without income

There is no long file to put together. Cover starts from a tax return, not from an application to the health insurance house.

  • Your identity document, to register in the SPV or at the counter.
  • Form 212, completed at Chapter II, the one for people who opt to pay CASS. It can be filed at any time of the year.
  • Payment of the first instalment, 25% of the contribution, on the day you file. Keep the proof.

The return is sent online through the SPV or on e-guvernare.ro with a qualified electronic signature, or on paper, at the registry of the tax office or by post with acknowledgement of receipt. If you do not have a computer, tax offices have self-service computers and staff can help you fill in the form.

If you pay for a spouse or a parent without income, complete Section 2 of the same chapter in your own return, once for each person. The amount is the same for each.

What health insurance without income costs in 2026

The amount does not depend on the month in which you file. The calculation base is the minimum wage in force on 1 January of that year.

For 2026, ANAF's calculation is: 6 × 4,050 lei = 24,300 lei, and 10% of this base is 2,430 lei. In ANAF's example, the first instalment is 608 lei, paid on filing, and the second, 1,822 lei, is due by 25 May of the following year. You can also pay the full amount at once. A rise in the minimum wage during the year does not change the amount, because the base remains the one from 1 January.

You can pay by card in the SPV or on Ghișeul.ro, by bank transfer, by postal order or at Treasury cash desks. ANAF warns that the option is final. If you move into an exempt category after filing, for example because you become pregnant, the contribution for that year is not recalculated.

12 months

When your cover starts and how to check it

CNAS grants insured status for 12 months from the date the Declarația unică is filed, not until the end of the calendar year.

For the following year you file a new return. You can check your status yourself, using your CNP (personal numeric code), in the CNAS insured-status checker. If you do not appear as insured after filing, contact the county health insurance house (casa de asigurări de sănătate județeană) and keep the proof of filing and payment to hand. The national health card does not need renewing along with your insurance: CNAS states that it is valid for 7 years.

Pachetul minimal

If you are not insured: what is free and what you pay

Even without insurance you are entitled to the pachet minimal (minimum package of services). It covers emergencies, not routine care.

  • Hospital: admission for life-threatening emergencies until the emergency is resolved, for diseases with epidemic potential and for childbirth.
  • Medic de familie (family doctor): one consultation for each emergency and at most two for each episode of acute illness.
  • Pregnancy and the postnatal period: the registration and monitoring consultations, plus two family planning consultations a year.
  • You pay in full for recommended tests, specialist consultations and medicines. The doctor cannot issue CNAS prescriptions or referral forms for you.

Questions about insurance without income

If you are not insured, do you pay for emergency care?

Not for the emergency itself. The pachet minimal covers hospital admission until a life-threatening emergency is resolved. You pay for treatment that is no longer urgent, and for the tests and medicines recommended after that.

Can I still be co-insured for free in 2026?

No. Spouses and parents without income have not been exempt since August 2025. The person who supports them can pay 2,430 lei in 2026 for each of them, through their own Declarația unică.

Can I pay CASS monthly?

No. The law provides for two instalments: 25% when you file the return and 75% by 25 May of the following year. You can also pay the full amount at once.

How long am I insured after filing the return?

12 months from the filing date. To stay insured, file a new return before that period ends.
