Chronic Disease & Ongoing Care Consultation

Ongoing, in-depth management for long-term conditions — not just today's symptom. Our Family Medicine doctors are registered on the Specialist Division in Family Medicine with the Irish Medical Council — the highest level of GP training available in Ireland. They are specifically trained to manage complex, ongoing, and multi-system conditions with the kind of clinical depth and continuity that chronic disease care actually requires. For a new or sudden symptom that appeared recently, our Acute Medical Consultation is more appropriate — same-day appointments available.

About this service

Who This Service Is For

This consultation is designed for patients who are already living with one or more diagnosed conditions and need structured, ongoing medical management — not a one-off assessment.

This is the right consultation if you:

  • Are managing a chronic condition such as hypertension, diabetes, asthma, or thyroid disease
  • Have recently received a diagnosis and need a comprehensive care plan
  • Are an expat or international resident without a registered GP in Ireland managing you long-term
  • Have multiple conditions that interact and need a doctor who understands the full picture
  • Want a doctor who will review your history, track your progress, and adjust your care over time
  • Need coordination across specialists, investigations, and ongoing treatment pathways

What Your Consultation Includes

Full Chronic Disease AssessmentYour doctor will review your diagnosis history, current condition status, medication, symptom control, and any changes since your last review — building a complete and current clinical picture rather than treating each appointment in isolation.

Personalised Care PlanBased on your assessment, your doctor will develop or update a structured care plan tailored to your condition, lifestyle, and long-term health goals — including monitoring targets, lifestyle recommendations, and clinical milestones.

Treatment ReviewA clinical review of whether your current management approach remains appropriate, effective, and safe given your current health status. Any adjustments are made at the doctor's professional discretion following full assessment.

Preventive Care & ScreeningChronic conditions increase risk across multiple systems. Your doctor will advise on appropriate screening, monitoring, and preventive measures relevant to your specific condition profile.

Coordination of CareWhere specialist input, diagnostic investigations, or in-person assessment are needed, your doctor will coordinate referrals and ensure continuity across your care pathway — so nothing falls through the gaps.

Clinical DocumentationReferral letters, clinical summaries, and other documentation issued at the doctor's professional discretion where clinically appropriate.

Why Patients Choose Us

IMC Specialist Division — Family MedicineOur chronic disease doctors are registered on the Specialist Division in Family Medicine with the Irish Medical Council. This is the highest level of GP qualification in Ireland and the standard you should expect for long-term condition management.

Built for expats and international residentsManaging a chronic condition in a country where you have no registered GP is genuinely difficult. We provide the ongoing primary care relationship that expats, international students, and international residents need — in their language, on their schedule, without the administrative barriers of the Irish GP registration system.

Continuity over timeWe maintain your clinical history, care plan, and consultation notes within our secure platform. Where possible we match returning patients with the same doctor — because continuity is not a luxury in chronic disease management, it is a clinical requirement.

20-minute appointmentsChronic conditions cannot be properly managed in 15 minutes. This appointment is structured to give your doctor the time your condition actually requires.

Questions

Frequently Asked Questions

What is the difference between this and an Acute Medical Consultation?

An Acute Medical Consultation is for new or sudden symptoms — something that appeared recently and needs assessment now. A Chronic Disease Consultation is for patients already managing a diagnosed long-term condition who need structured ongoing care, a care plan, and a doctor who will engage with their full clinical history rather than just the presenting complaint.

Do I need to be registered with a GP in Ireland to use this service?

No. This service is specifically designed for patients — including expats and international residents — who do not have a registered GP in Ireland managing their chronic condition. You do not need a PPS number or prior Irish GP registration to access care.

Can I use this service if I was diagnosed in another country?

Yes. Bring any relevant medical records, test results, or clinical letters from your previous doctor. Your Global Health doctor will review these in full and build your care plan from your complete history, not just what is available in the Irish system.

Are the doctors on this service different from standard GPs?

Yes. Chronic Disease consultations are conducted by doctors registered on the Specialist Division in Family Medicine with the Irish Medical Council — meaning they have completed postgraduate specialist training in primary care beyond standard GP registration.

What if my condition requires specialist care beyond GP level?

Your doctor will advise you clearly if your condition requires specialist input and will coordinate a referral to the appropriate service. Our role is to manage what can be managed at GP level and to ensure you get to the right specialist efficiently when you need one.

Is this service suitable for children with chronic conditions?

Yes. Family Medicine consultations are appropriate for patients of all ages, including children managing ongoing conditions such as asthma, eczema, or diabetes.

Medical disclaimer. All services are provided at GP level in accordance with Irish healthcare regulations. Prescriptions, referrals, and medical certificates may be issued only when clinically appropriate and at the doctor's discretion. Our doctors do not routinely prescribe controlled substances through online consultations. Electronic sick leave certificates are not accepted by the Department of Social Protection in Ireland. Backdated sick notes are not routinely issued due to the lack of a direct clinical assessment at the time of illness.
