How This Consultation Works
The medical assessment for most of these certificates requires review of specific parameters — usually blood pressure, visual acuity, and sometimes hearing acuity — which must be documented before your consultation.
Before the consultation, you should have available:
- A recent blood pressure reading — this can be taken at a pharmacy, health centre, or with your own validated device
- A recent visual acuity test result, where relevant to the type of certificate
- A history of any medical conditions relevant to the purpose of the certificate
During the consultation, your doctor reviews these parameters, together with your full clinical history, and determines whether you meet the fitness criteria required for the intended purpose. When the criteria are met, the certificate is issued electronically.

Types of Certificate Available
Sports Medical Certificate. For federated or non-federated sport, required by sports federations, gyms, and clubs in Portugal. Assesses cardiovascular and general physical fitness for the intended activity. Usually required annually for federated sport.
Hunting Licence Medical Certificate. Required by the ICNF for obtaining and renewing a hunting licence. Assesses physical fitness, visual and hearing acuity, and the absence of conditions that would compromise safe handling of a hunting firearm.
Recreational Boating Licence Medical Certificate. Required by the National Maritime Authority for obtaining a recreational skipper's licence. Assesses general physical fitness, and visual and hearing acuity, relevant to safe navigation.
Medical Certificate for Public Competitions and Civil Service. Required in civil service admission processes and certain regulated professions. Assesses general physical and mental fitness to perform the role, in line with the requirements of the competition or employer.
School and Physical Education Medical Certificate. For exemption from or restricted participation in physical education, or for enrolment in school and university sports activities.

What Your Consultation Includes
Clinical History Review. Your doctor reviews your full medical history, with particular attention to conditions relevant to the specific purpose of the certificate requested.
Assessment of Documented Parameters. Review of blood pressure, visual acuity, and other relevant parameters, based on the results you present, in the context of the fitness criteria required by the relevant authority.
Fitness Determination. Based on the complete assessment, your doctor determines whether you meet the medical fitness criteria for the intended purpose.
Certificate Issuance, When Appropriate. When the criteria are met, the medical certificate is issued electronically, ready for submission to the relevant authority.
Referral, When Necessary. If further tests, specialist assessment, or a psychotechnical test are needed before the certificate can be issued, your doctor clearly informs you of the next steps.

- Issuance of the certificate depends on the clinical review of your documented health parameters and medical history — it cannot be guaranteed in advance
- You must have recent measurements of the parameters relevant to the type of certificate requested
- Different certificate types have different fitness criteria — your doctor assesses according to the specific stated purpose

- Certificates for occupational health purposes may require assessment by an occupational health doctor, outside the scope of this service
